To book with UniSalute you can contact Polispecialistico Lambrate directly and make a first appointment for a visit, teeth cleaning or prevention package.
You will then need to contact UniSalute to communicate the date of the scheduled appointment. Or you can request the service you need directly from the UniSalute portal, then Polispecialistico Lambrate will contact you to make an appointment.
The fund will notify you by e-mail of the details of the amount reimbursed.

You will then have to request the opening of the file directly to the fund by communicating the requested service and the appointment date agreed with the Center.

To book with Assirete, Polispecialistico Lambrate will fill in the quote for the treatment plan using the specific form and deliver the documentation that you will need to send, through the appropriate Assirete channels, to activate the treatment.
Assirete will send the authorization to proceed with the treatment to the Center only after you have sent him the request for a quote. The maximum amounts and any overdrafts to be paid by the insured will also be indicated.
